Appointments
We see patients by appointment only. Please call to schedule your routine appointments as well as urgent same day appointments when needed. You may print and fill out our forms prior to your appointment to save time when checking in, especially if you are a new patient. As a courtesy, we do give appointment reminder calls the day before your appointment. If you must cancel your appointment please give us as much advanced notice as possible, but at least 24 hours notice. We do realize that there are certain circumstances when 24 hours notice is not possible. We also mail out appointment reminders to our patients for annual exams to help you plan out your care.
After Hours Emergency Care
If you have an emergency, you must go to the emergency room. The on-call doctors are accessible 24 hours a day. Please keep in mind that the on-call doctor will return your call as soon as possible that this may not be immediate due to the possibility of caring for other patients in the hospital. If a return call is not received in a ½ hr, please call our office again.
Insurance
Our office accepts most insurances. We do offer a discount on self-pay patients. Some insurances do require a referral from your primary care provider prior to your visit and some insurance plans may not cover certain medical services. Please check with your insurance carrier or call our office.
Prescription refills
You may call or have your pharmacy call for prescription refills. Please leave a message with the staff including your pharmacy name and phone number. Please allow at least 24-48 hours to allow us and the pharmacy to process your request, so that you do not run out of your medicine.
Medical records
We are happy to provide you with copies of your medical record at any time. In order to obtain your records, we will need written authorization to obtain a "Release of Records" form or download the form here.
